MOTHER_ID Character (or numeric if possible) Patient ID of mother of the child
PREG_SEQ numeric Sequence number of the pregnancy for the specified mother
CHILD_ID Character (or numeric if possible) Patient ID of the child (if child is not enrolled into care, enter mother's ID with dashed numeric Suffix such as [MOTHER_ID]-1, [MOTHER_ID]-2, etc. here)

  • 1=Born alive, HIV negative

  • 3=Born alive, unknown HIV Status

  • 10=Stillborn
  • 11=Spontaneous miscarriage
  • 20=Termination: surgical
  • 21=Termination: medication
  • 22=Termination: method unknown
